I work at a referring physician’s office. How do I refer a new patient?
Please fax over the referral or email it to our New Patient Coordinators through secured messaging at NPC@APM-AZ.com. To see if your patient needs authorization, please click here. We will also need the last 3 office notes, demographics, and and X-Rays or MRI’s. Once we receive your referral, we will contact the patient to schedule their consultation.

Can I fill out my New Patient Paperwork prior to my appointment?
Yes, please click on Patient Intake Forms under the New Patient tab to access them. You may bring them to your first appointment or email them to our New Patient Coordinators so they’re in your chart when you arrive.
